Refer a patient to a Praxi practice
This page is for referrers — GPs, psychiatrists, paediatricians, NDIS plan managers, EAP coordinators, schools, and any other professional sending a patient or client to a practice that uses Praxi.
Every Praxi practice can publish their own referrer page at
https://app.praxi.au/refer/<their-slug> with practice-specific pathways and
contact details. The information below describes the pathways available across
the platform — the practice you're referring to chooses which ones they
accept.
The fastest pathway: online referral form
Most Praxi practices accept referrals via a public online form at
https://app.praxi.au/enquire/<practice-slug>. The form:
- Takes 2–3 minutes to complete.
- Captures patient demographics, presenting concern, funding source, and urgency.
- Lands directly in the practice's intake pipeline.
- Triggers the practice's automation — they're notified immediately if you flag the referral as urgent.
You'll typically get an automated acknowledgement within minutes, and a clinical response within the practice's stated SLA (often 1–3 business days).
Other pathways
Depending on what the practice has enabled, you may also be able to refer via:
Secure email
The practice publishes a practice-configured intake email address. Send your referral letter as you would to any other clinical inbox. The practice's team reviews it and adds the referral.
HealthLink / Medical-Objects
If you already use HealthLink, Argus, or Medical-Objects EDI messaging, the practice can give you their EDI code. Referrals sent this way appear in their intake pipeline alongside online and email referrals.
Fax
For practices that still accept fax, the number appears on their public page. Faxes are typically scanned and ingested manually within one business day.
Phone
Speak directly to the practice's intake team for urgent referrals or to discuss complex cases. The phone number is on the practice's public page.
Referrer Portal access
For frequent referrers (e.g. you send several patients a month), the practice can set you up with Praxi Referrer Portal access. The portal lets you:
- Track every referral you've sent.
- See outcomes once consent is given.
- Read progress reports the practice has shared.
- Submit new referrals with patient details pre-filled.
Portal access is arranged by the practice — contact them directly to be set up. You'll receive an invitation email with the portal link, and you sign in with a one-time code sent to your email.
What information should I include?
A complete referral typically includes:
- Patient details: name, date of birth, contact info (phone, email), Medicare/DVA card (if applicable).
- Funding source: Medicare MHCP, NDIS, EAP, WorkCover, TAC, or private.
- Presenting concern: brief clinical context — what's bringing them to therapy?
- Risk indicators: any safety concerns, recent hospital admissions, current risk to self or others.
- Urgency: urgent (within 1 week), soon (within 2-4 weeks), or routine.
- Your details: your name, provider number, practice, contact info.
- Mental Health Care Plan (for Medicare MHCP): if you've prepared an MHCP for the patient, please attach it.
What happens after I refer?
- Acknowledgement — usually instant for online forms; within one business day for email/fax.
- Triage — the practice reviews the referral within their SLA (typically 1–3 business days for routine, 24 hours for urgent).
- Decision — accept, waitlist, or decline (with reason).
- Contact — if accepted, the practice contacts the patient directly to schedule the first appointment.
- Follow-up — if you've signed up to the referrer portal, you'll see status updates and (with consent) outcome reports.
What if I haven't heard back?
Most practices respond within their stated SLA. If you haven't heard:
- Check the practice's published wait times on their
/refer/page — they may explicitly state when to expect a response. - Contact the practice directly via phone — sometimes referrals get delayed by funding-source verification (e.g. Medicare card lookups).
- Resend via a different pathway if you suspect the original didn't arrive (rare, but it happens with fax).
Privacy
Referrals are PHI and are handled under the Privacy Act 1988 (APP 11.1): encrypted at rest, transmitted over TLS, and only accessible to the practice's authorised staff. Praxi does not share referral data with any third party.
