Client Documents

Praxi allows you to upload, organise, and share documents for each client. Documents are stored securely with encryption at rest and access-controlled by role.

Uploading Documents

  1. Open the client's profile
  2. Go to the Documents tab
  3. Click Upload
  4. Select the file from your computer or drag and drop it into the upload area
  5. Choose a category for the document (see below)
  6. Add an optional description
  7. Click Upload

You can upload multiple files at once. Each file is scanned and stored securely.

Supported File Types

TypeExtensions
DocumentsPDF, DOCX, DOC, TXT
ImagesPNG, JPG, JPEG

Maximum file size is 10 MB per file.

Document Categories

The Documents tab consolidates all document types into one searchable browser. Use the filter chips at the top to narrow the view:

Filter chipContents
All FilesEverything on the client record
DocumentsGeneral uploads, correspondence, client-provided items
Clinical ReportsClinical assessment reports
NDIS ReportsProgress reports and NDIS plan documents
LettersGenerated letters to GPs, schools, and other parties
FormsCompleted intake forms, assessments, and questionnaires
Referral DocumentsGP referral letters and supporting documentation

Within a category, you can add a free-text subcategory when uploading (for example "Consent forms" or "Correspondence") to keep large records organised.

Sharing Documents with Clients

Share documents with a client via the Client Portal:

  1. Open the document from the client's Documents tab
  2. Click Share with Client
  3. The document becomes visible in the client's portal under their documents section for the share period you choose (7 days by default, up to 90 days)

To stop sharing early, click Revoke Access. The document is immediately removed from the client's portal view.

Consent Before Sharing

Ensure you have the client's consent before sharing clinical documents. Praxi logs all share and revoke actions in the audit trail.

Document Retention

Praxi follows document retention requirements under the Privacy Act 1988:

  • Client documents are retained for a minimum of 7 years from the date of last contact
  • Documents for clients who were minors at the time of treatment are retained until the client turns 25 years of age, or 7 years from last contact, whichever is longer
  • Archived client records and their associated documents remain accessible to authorised staff
Right to Erasure

Clients may request deletion of their personal information. Erasure requests are managed through the privacy compliance workflow and are subject to regulatory retention requirements.

When a client is archived, their documents are preserved but no longer appear in default views. They can be accessed by searching for the archived client record.
