Client Discharge and Transfer
When a client completes treatment or transfers to another provider, Praxi guides you through a structured discharge process that produces a clinical discharge summary, an optional referral letter, and a secure record-transfer package.
Initiating discharge
- Open the client's record
- Click the Actions menu (three dots) in the client header
- Select Discharge Client
- Complete the discharge form:
- Discharge date
- Discharge reason — Treatment complete, client-initiated, transferred, relocated, non-attendance, or other
- Discharge summary — A clinical summary of treatment provided, outcomes, and recommendations. Use the AI Draft button to generate a starting point based on the client's notes and assessment data, then review and edit
- Click Continue
Discharge summary
The discharge summary is a key clinical document. It covers:
| Section | Content |
|---|---|
| Treatment overview | Duration, number of sessions attended, therapeutic approach |
| Presenting issues | Original referral reasons and diagnoses |
| Progress | Clinical progress, assessment score changes, goals achieved |
| Recommendations | Ongoing care suggestions, relapse prevention, follow-up timing |
| Referrals | Receiving provider details (if transferring) |
Once saved, the discharge summary is attached to the client's record and can be shared with a receiving practitioner subject to client consent.
Generating a referral letter
If the client is transferring to another provider:
- During discharge, click Generate Referral Letter
- Enter the receiving practitioner's details (name, practice, address)
- Praxi generates a letter including relevant clinical history, current medications, assessment results, and recommendations
- Review and edit the letter
- Send via secure message, email, or print for posting
Record transfer
To send clinical records to the receiving practice:
- Navigate to the client's Documents tab
- Click Prepare Transfer Package
- Select which documents to include — clinical notes (all or selected), assessment results, treatment plans, referral letter, consent records
- The system creates a secure download link or encrypted PDF package
- Share the link with the receiving practitioner — links expire after 7 days
Consent required
Client consent is required for record transfer. Praxi will prompt you to record consent if none is on file before it generates the transfer package.
Reactivating a discharged client
If a discharged client returns for treatment:
- Search for the client in Care > Clients — discharged clients appear with a "Discharged" status badge
- Open the client's record and select Reactivate
- The client's status returns to active and all previous records remain accessible
Tips
- Complete the discharge summary within 7 days of the last session
- Use the AI draft to save time — review and edit to ensure clinical accuracy
- Always obtain written consent before transferring records to another provider
- Praxi retains discharged client records for 7 years as required by Australian law (Privacy Act 1988)
