Clinical Notes on iOS

The Praxi iOS app supports full clinical note management, including creation, filtering, voice dictation, AI-assisted drafting, and the three-phase session interface with live transcription.

Note List

The Clinical Notes screen shows all your notes with filtering and sorting options:

  • Filter chips -- Quickly filter by status: All, Unsigned, or My Notes.
  • Sort options -- Sort by date (newest first), client name, or status.
  • Urgent signing banner -- When notes are approaching the AHPRA 24-hour signing deadline, a red warning banner appears at the top of the list showing how many notes need attention.

Each note row displays the client name, note type, status badge, content preview, and creation date. Notes with overdue signing deadlines are highlighted with a red border.

24-Hour Signing Requirement Under AHPRA guidelines, clinical notes must

be signed within 24 hours of the session. Praxi displays a reminder badge on unsigned notes and sends a push notification as the deadline approaches. :::

Creating a Note

  1. Tap the + button in the top toolbar of the Clinical Notes screen, or open an appointment and tap Add Note.
  2. Select a note template (e.g., Progress Note, Initial Assessment, SOAP).
  3. Fill in each section using the structured editor or free-text fields.
  4. Tap Save as Draft to continue later, or Finalise when complete.

Quick Signing

Swipe left on any finalised note in the list to reveal the Sign action. Swipe fully to sign the note immediately, or tap the button to confirm.

Session Interface (Prepare, Active, Wrap Up)

For in-session note taking, Praxi provides a three-phase session workspace:

  1. Prepare -- Review the AI-generated pre-session brief, client history, Medicare session tracking, and previous notes before the client arrives.
  2. Active -- During the session, live voice transcription captures the conversation in real time. The transcript appears alongside AI-generated live notes that update progressively. Clinical guidance cards may appear with relevant suggestions. A confidence indicator shows the AI's certainty level.
  3. Wrap Up -- After ending the session, review the AI-generated clinical note side-by-side with the full transcript. Edit sections, verify flagged content, and finalise the note.

A phase indicator at the top of the screen shows your progress through the three stages.

Voice Transcription During the Active phase, the app streams audio to

the Praxi transcription service for real-time transcript generation. Use a quiet environment and speak clearly for best results. Clinical terminology (DSM-5 codes, medication names, assessment tools) is recognised automatically. :::

AI Note Assistance

If your subscription tier supports AI features, Praxi can generate a structured draft based on the session transcript and your selected template. AI-generated content is marked with a confidence indicator, and sections flagged with [VERIFY] require your clinical judgement before finalising.

Templates on Mobile

Templates adapt to the mobile screen with collapsible sections. Each section (e.g., Presenting Issues, Interventions, Plan) is displayed as an expandable card. Required sections are marked and must be completed before finalising.

Voice Dictation

Tap the microphone icon in the note toolbar to open the voice transcription view. Dictate your clinical note and the transcription is streamed in real time. This is separate from the live session transcription -- it is designed for quick dictation when writing up notes after a session.

Offline Drafts

Note drafts can be created and saved without an internet connection. Drafts are stored securely on-device and synchronised when connectivity is restored. Finalising and signing requires an active connection.
