Each template carries required and optional sections appropriate to the report type and audience.

Report Sections

Each report template comes with required and optional sections. Here are typical sections for a Comprehensive Psychological report:

Required sections: Client demographics, referral information, presenting problem, personal history, family history, education history, occupational history, medical history, psychiatric history, mental state examination, psychometric summary, clinical formulation, DSM-5 diagnosis, treatment recommendations, practitioner signature.

Optional sections: Relationship history, substance use history, trauma history, current circumstances, clinical observations, cognitive assessment, functional assessment, risk assessment, differential diagnosis, prognosis, confidentiality notice, methodology statement.

Court reports and forensic assessments include additional required sections such as methodology statement and confidentiality notice.

AI-Assisted Drafting

Praxi's clinical AI assistant can draft individual sections of your report from session notes and assessment data. Rather than generating the entire report at once, you work through it section by section:

  1. Open the client record, open the More menu (⋯), and select All reports, then click New Report
  2. Choose the report type (e.g. Comprehensive Psychological)
  3. The report is created with all sections laid out
  4. For each section, click Generate -- Praxi drafts content from the client's clinical record
  5. Review the generated text and its status indicator
  6. To adjust a section, use Refine with your feedback -- Praxi will revise the content based on your instructions
  7. Edit sections directly in the rich text editor as needed

The diagnosis, formulation and recommendations in a report are your clinical judgement. Praxi drafts from what is in the record for you to check and edit. It never generates alternative hypotheses (differential diagnoses). That section holds only what you write.

Generation Preferences

When generating sections, you can set preferences for:

PreferenceOptions
PerspectiveFirst person or third person
FormalityFormal or semi-formal
Target audienceGP, specialist, court, or client

These preferences are set at the report level but can be overridden per section.

Section Status Tracking

Each section shows its current status:

StatusMeaning
EmptyNo content yet
AI draftGenerated by Praxi, awaiting your review
ReviewedYou have reviewed and edited the content
FinalLocked as part of a finalised report
Refining AI Drafts

When using Refine, you can choose to either preserve the existing content and improve it, or rewrite it entirely based on your feedback. Use the Preserve content option to keep the overall structure while adjusting specific details.

Finalising Reports

Once your report is ready:

  1. Complete all required sections (the report cannot be finalised with empty required sections)
  2. Click Finalise to lock the report
  3. All sections with content are marked as final
  4. The report is locked and cannot be further edited
  5. Export as PDF or DOCX for sharing
warning

Finalised reports cannot be modified. Review all sections carefully before finalising. A progress indicator shows the percentage of required sections completed.

Exporting Reports

Export finalised or in-progress reports in two formats:

FormatBest For
PDFSharing with third parties, printing, filing
DOCXFurther editing in Word

Export options include:

  • Include letterhead -- Adds your practice header and branding
  • Include page numbers -- Adds page numbering to the footer
  • Watermark -- Adds a watermark overlay (PDF only)
Court Reports

Court reports (PNCR 1-2017) are automatically paragraph-numbered in the generated document, following the standard forensic report format.

Who can use psychology reports

Practitioners (and interns, under supervision) can view, create, edit, generate with AI, finalise, and export psychology reports for their own clients. Clinical supervisors and practice owners have practice-wide access to reports across the practice. Clinical governance has read-only oversight of reports across the practice.
