Session Memory

Session Memory provides AI-powered continuity between client sessions. When a practitioner finalises a clinical note, Praxi automatically creates a structured memory entry that captures key facts, themes, and clinical observations. These memories are retrieved at the start of the next session to provide relevant context.

Automatic Memory Creation

Memory entries are created automatically when a clinical note is finalised:

  1. The practitioner completes their review and finalises the session note
  2. Praxi analyses the note and extracts key information: a summary, therapeutic themes, interventions used, treatment goal progress, risk level, and homework status
  3. A memory entry is created and linked to the client record
  4. If the note content is modified during the signing review, the memory is automatically re-consolidated when the note is signed

No additional action is required from the practitioner. Memory creation happens in the background after note finalisation.

What Gets Stored Memory entries contain structured clinical facts

extracted from signed notes. They do not store verbatim transcripts or raw session audio. The source note is always linked for reference. :::

Theme Tracking

Praxi tracks clinical themes across sessions and monitors their progression:

Theme StatusMeaning
EmergingTheme has appeared in one session
ActiveTheme has recurred in two or more sessions
ResolvingActive theme has not appeared for three or more consecutive sessions
ResolvedTheme has not appeared for five or more consecutive sessions

Themes are surfaced in the session context panel so you can see patterns across the treatment journey. Theme progression updates automatically as new notes are signed. Similar themes are automatically merged when detected.

Memory Retrieval for Next Session

When you open a new session for a client, Praxi retrieves relevant memories:

  1. Click Start Session from the client's appointment
  2. The session context panel displays relevant memories from previous sessions
  3. Praxi retrieves the five most recent sessions plus up to three additional thematically related sessions (maximum eight total)
  4. Memories are ranked by multi-factor relevance scoring that considers theme overlap, recency, frequency, and clinical importance (e.g., sessions with risk indicators are ranked higher)
  5. Active themes and unresolved plan items from previous sessions are highlighted

This gives you a quick summary of where the client left off without needing to re-read all previous notes.

Reviewing Full Notes Session memory provides a summary, not a replacement

for reading clinical notes. Click any memory entry to open the linked source note for the full record. :::

Privacy Controls

Session memory respects client consent and practitioner preferences:

  • AI processing consent required -- Memories are only created if the client has consented to AI processing of their clinical data
  • Practitioner-only access -- Memories are visible only to the treating practitioner (and supervisors with appropriate access)
  • Not visible in Client Portal -- Session memories never appear in the client-facing portal

Managing Memories

To view a client's session memories, open the client's profile or access memories from within an active session. You can browse memories chronologically or by theme, and click a memory to view its details and linked source note. If a memory is inaccurate, correct the underlying clinical note via an addendum -- memories re-consolidate from the signed note.

Subscription Requirements

Session Memory is available on PRO and ENTERPRISE subscriptions. ESSENTIALS subscribers can use voice transcription but do not have access to the session memory and theme tracking features.
