Couples & Group Notes
Praxi provides specialised note-taking workflows for couples therapy and group sessions. These workflows address the unique consent, privacy, and documentation requirements of multi-client sessions.
Consent Models for Couples Notes
Before writing couples notes, you must configure the consent model for each couple. Praxi supports three models:
| Model | Description |
|---|---|
| No secrets | All session content is shared between both partners. Neither partner can request information be withheld from the other. |
| Secrets | The practitioner may hold individual disclosures confidentially. Information shared in individual contact is not included in shared notes. |
| Case by case | The practitioner decides on a case-by-case basis whether specific information should be shared or held confidentially. |
Set the consent model when creating the couple record, or update it from the couple's profile under Consent Settings. Both partners must acknowledge the selected model.
during treatment, both partners must consent to the new arrangement. Praxi records the change and the date it took effect in the audit trail. :::
Writing Couples Notes
When creating a note for a couples session:
- Open the session and click Create Note
- Praxi shows the couples note flow, where you can select a couple relationship or choose two individual clients
- Both clients are displayed in the note header
- Write the shared section covering joint observations, relationship dynamics, interactions, and interventions
- If the consent model allows, add individual sections for each partner. Under the secrets or case by case models, these individual sections are only visible to the practitioner and the relevant partner
- With the case by case model, you can set the visibility of each section individually (shared, partner A only, partner B only, or practitioner only)
- Sign the note as usual
Shared couples notes require the relationship's consent to be granted, active, and unexpired. If the recorded consent has expired, Praxi blocks creation of a shared note until consent is renewed.
Individual sections are clearly marked and access-controlled. Each partner only sees their own individual section when viewing notes through the Client Portal (if note sharing is enabled).
Per-Member Notes in Group Sessions
Group therapy notes require a separate note for each participant to protect confidentiality. Every attendee must have a signed confidentiality agreement recorded -- Praxi blocks group note creation if any participant is missing one. Shared content is also checked automatically: Praxi rejects shared sections that name individual members, and individual sections that reference other group members.
- Open the group session and click Create Notes
- Praxi shows the group note flow, where you select the group and mark attendance (who was present)
- Write shared content that applies to all attendees (e.g., session theme, group activities)
- Optionally use Copy Shared Content to distribute the shared section to all member notes at once
- Write each participant's individual note, documenting:
- Their participation and engagement
- Individual observations and progress
- Relevant interactions (without identifying other group members)
- Plan for ongoing treatment
- Optionally enable individual member sections for per-participant notes within a single view
- Sign each note separately
De-Identification for Group Notes
Praxi enforces de-identification in group notes:
- When referencing interactions between group members, use role descriptions rather than names (e.g., "another group member" rather than a specific name)
- The AI note generation feature automatically de-identifies other participants when generating per-member notes from session transcription
- Each member's note is stored independently and is only accessible from that client's record
session, Praxi generates individual de-identified notes for each participant. Review each note carefully to ensure no identifying information about other group members has been included. :::
Supervision Requirements
Couples and group work may have additional supervision requirements depending on your registration and endorsement:
- Document supervision discussions related to couples or group sessions in your supervision notes
- Praxi links session notes to supervision records when you reference them during supervision
- Interns and provisionally registered practitioners should discuss all couples and group sessions with their supervisor, as reflected in the supervision module
