Completing Tools From Your Practitioner
Your practitioner can send you a range of things to complete between sessions — forms, worksheets, safety plans, mood trackers, body charts, guided practices, and learning modules. Everything they assign appears in one place: My Plan, under Wellbeing in your portal menu.
Where to find your tools
- Sign in to your portal.
- Tap My Plan under Wellbeing in the menu. Everything your practitioner has asked of you is listed there, ordered by what's due next, with a label showing what kind of activity each item is.
- Each item shows a short title, a due date if your practitioner set one, and any message they included with it.
- Tap any item to open it.
Whatever kind of activity it is, the tool itself will guide you through completing it.
The four ways your practitioner may ask you to complete something
Your practitioner decides how each tool is completed. You might see any of these styles:
On your own
This is the most common. You open the tool in your portal, fill it in when you have time, and submit. Your answers save automatically as you go, so you can close the app and come back later.
Together, in a session
During an in-person or telehealth appointment, your practitioner may fill something in while you watch. You see it live on your screen (read-only). You can also leave a short note alongside it if something comes up.
Together, both typing
Some tools let you and your practitioner type in the same document at the same time. You will see a small icon showing who is editing which field — if your practitioner is typing in a box, you wait a moment and it will unlock.
For your practitioner's notes
Some tools are for your practitioner to complete in their own records. These do not appear in your portal — you will not see them.
What each kind of tool looks like
- A form or worksheet — questions to answer, one screen or a few. Tap Submit when done.
- A questionnaire — a short set of standard questions your practitioner uses to understand how you are going.
- A daily tracker (mood, sleep, food, pain) — a short entry each day. You will get a reminder on your phone or by email at the time of day your practitioner picked.
- A body chart — a picture of the body you can tap on to show where something is happening, then describe it.
- A plan (safety plan, home exercise plan) — a series of sections or steps. You work through them in order and submit once at the end.
- A guided practice — breathing, grounding, meditation and similar practices, with audio or on-screen guidance.
- A learning module — a short course with videos, reading, and quick questions. Complete at your own pace.
- A programme — a structured series of activities over days or weeks.
If you are unsure
If a question is unclear, leave it blank and ask your practitioner at your next appointment, or send them a secure message. You will not get into trouble for leaving something blank.
If you miss a due date
Overdue items are flagged but you can still complete them unless your practitioner has removed them. If in doubt, just do it — your practitioner will decide what to do with a late submission.

Everything you submit is private between you and your practice. Your practitioner can see it; other staff can only see it if they are involved in your care. See Privacy and Security for more.
