Consent Forms

Consent forms allow your practitioner to provide treatment and share information where necessary. Australian health regulations require that your consent is reviewed periodically.

  • Treatment consent — Permission for your practitioner to provide care
  • Information sharing — Permission to share information with other professionals (e.g., your GP)
  • Recording consent — Permission to record sessions for note-taking purposes
  1. Read the form carefully
  2. Ask your practitioner if you have questions
  3. Tick the consent boxes you agree to
  4. Sign and submit

Managing your consents

You can view all your consents in the Consents section of the portal. Each consent shows:

  • What you have agreed to
  • When it was given
  • When it expires

Consents are valid for 12 months. When a consent is approaching its expiry date, you will see it listed as needing renewal. To renew:

  1. Go to Consents in the portal
  2. Find the consent marked as expiring
  3. Review the details and tap Renew Consent

You can withdraw consent at any time directly from the portal:

  1. Go to Consents in the portal
  2. Find the consent you wish to withdraw
  3. Tap Revoke and provide a reason
  4. Confirm that you understand the implications

Withdrawing treatment consent may affect your scheduled appointments. Withdrawing consent will not affect the care you have already received.
